- Authentication: Your drivers License or Medical Certificate of Death, Death Certificate, Proof of Death Certificate (also known as a Funeral Director’s Statement of Death by funeral home)
- A Credit Card: to pay the fee (No need for 30 Day plan)

- Live Stream Links (you can have two links like Youtube and Facebook)
- Contact Persons Full Name and Telephone Numbers. (Two person can be added)
- Any additional Information (This can be added at the bottom of the form. Including links and names.)
You can always login and edit your form anytime after submission.
